Scrotal tumors can be classified as intratesticular tumors, such as testicular cancer; and extratesticular tumor, such as leiomyomas that may originate from any body structure containing smooth muscle.

- Painless or painful bulge or lump noted within the scrotum
- A feeling of heaviness in the scrotum
- A dull ache in the lower abdomen or in the groin
- Enlargement or tenderness of the breasts
- Infertility may occur

- A physical examination shows a mass within the scrotum.
- An ultrasound evaluation of the scrotum
- A biopsy of mass
- Blood test: Alpha-fetoprotein, testosterone, human chorionic gonadotropin, estriol, 17-ketosteroids may be altered

- Undescended testicle
- Inguinal Hernia during childhood
- Previous testicular cancer of the other testicle
